The Personnel Action Form (PAF) is the form faculty submit when applying for Reappointment, Tenure, or Promotion.
Below is important information to all full-time junior faculty who must fill out the PAF every year until tenure, as well as faculty going for a promotion. PAFs are submitted to Department Chairs in October. Please note that all deadlines for submission are firm.

The Early Alert System: If you observe a student is experiencing academic difficulty or dealing with other problems, you can flag the student for early intervention. To submit an Early Alert please follow these instructions:
- Go to signon.stjohns.edu.
- Select the "New UIS Experience" application.
- Find the Faculty Grading (Non-Law) Tile and click Faculty Early Alert.
- If you do not see that tile, click the menu icon in the top left, choose Discover, and type into the search. You may then add it to your home view.
- The term will show as Fall 2024.
- Select the section the student is enrolled in. *
- Select the student. *
- Follow the prompts to provide information about the situation.
- Note that there is the option to notify the student that you've submitted an early alert. Use your discretion when choosing this option.
- Once you have completed the form, click submit.
- After submitting an Early Alert, you will receive a confirmation email that the Alert was received by the system.
*You may see a "Please Wait" notification at these steps. Once the notification goes away, you may select the option.
The recipient of the Alert will contact the student to address the reported issue(s) and collaborate with the student on devising an action plan to resolve the matter. The student will be encouraged to share a summary of the action plan with you; you will receive follow-up from the recipient of the Alert as well.
The University Learning Commons is located in the Library, Room 103 (next to Einstein Bros. Bagels). Students can schedule an appointment with a tutor by visiting the office or calling 718-990-1367. Students can now make online appointments on the University Learning Commons website.
The University Writing Center is located in St. Augustine Hall, Room 150. Students can schedule an appointment online or drop-in for a session with a peer tutor. They can also receive support through online chat sessions or by emailing a paper. Visit the Writing Center website to access the scheduling system.
University Library: Students may consult with librarians one-on-one in person, online through the Ask Us! website, or via text at 1-917-652-6936. Hours of operation for the physical facility and extensive electronic databases are available at the University Libraries website.
Every full-time faculty member is required to complete the Annual Faculty Activity Report (AFAR) describing their activities in the areas of teaching, research and scholarship, service, and professional development for the preceding year.
The Office of the Provost is currently revising the procedure for new program proposals. Before you initiate the process of any of the following changes, please contact your Dean with information about your plans so they can provide you with the appropriate information.
- creating a program, advanced certificate, or pathway
- adding distance learning to an existing program
- moratorium or discontinuing a program
- adding concentrations or minors to a program
- changing curriculum
- renaming a program
Occasionally, opportunities arise to form partnerships. If you believe you have such an opportunity, please review the attached PowerPoint then complete the Initial Proposal Form and return it to your Dean for first review before you enter into substantive discussion with another party.
Partnerships go through several stages of review and approval. These preliminary questions will determine if the potential partnership can advance to the next stage, the Dean’s Council. If approved there, you will receive a more detailed template with a P&L statement to complete and submit to the Office of the Provost for final review.
Note: Partnerships will be approved and prioritized only if they align with the University Strategic Plan and the annual academic priorities set forth by the Provost and the Dean in their respective memos, so please keep this in mind when deciding whether or not to move forward.
Examples of prioritized initiatives under the current strategic plan:
- student success partnerships - with priority given to those providing internships or externships leading to career outcomes
- institutional financial sustainability partnerships - with priority given to cohort models offering skill-based programs to industry partners
The title of Emeritus may be granted to a faculty member who has served at least ten (10) years at the University with at least ten (10) years in the rank of Associate Professor or Professor and who has been retired from full-time University service for at least one year. Specific action by the Office of the President is required.
Recommendations of faculty members emanate from the departmental/divisional faculty acting as a committee of the whole and are forwarded to the Dean. The Dean forwards his or her recommendations to the Provost who makes a recommendation to the President.
Individuals recommended must have been honorably retired and are not teaching or associated in a full-time capacity with another educational institution. All recommendations must include the specific achievements of the individuals, in the areas of scholarship or creative work, teaching recognition, administrative contributions or other significant service to the University and the profession. It is expected that appointment to Emeritus will recognize a national record of professional achievement, a life of respect for the rights and dignity of every person, and a strong commitment to the University’s mission.
The Emeritus application opens in June each year and closes in December. Please contact your Dean's office with any questions.
