Student Conduct Checks

The Office of Student Conduct provides information about a student's conduct history to interested third parties with a legitimate educational interest for making the request.

If you are requesting information about a student's conduct history, please submit this form using the attached Excel template for collecting student information. Once the request is sent, please allow the Office of Student Conduct up to three (3) business days to complete your request.

Please note: The Office of Student Conduct provides standard language that MUST be included the initial application, confirming that eligible students have the right to have some control over the disclosure of personally identifiable information from their education records. You must also retain a record of the student affirming that they are allowing the Office of Student Conduct to share this information with you. If you would like a copy of this standard language, please contact the Office of Student Conduct.

Attention St. John's Students: If you are requesting information about your conduct history for a transfer application or at the request of an external organization, do not complete this form. You must contact the Office of Student Conduct directly to make this request.

Records Requests

The Office of Student Conduct is responsible for maintaining certain student records; please allow up to three (3) business days for this request to be completed. Failure to properly complete these forms or provide signed authorization will result in a delay in processing. Incomplete requests may not be processed.

Any student who is completing the “Common Application” for transfer out of St. John’s to another institution must complete the Transfer College Report form.

Students requesting this information must complete the Transfer College Report form and sign it before this request can be processed. Please make sure you have completed all necessary requirements prior to submission.

Students who are requesting a statement of their conduct history for graduate programs or for employment purposes must complete the Dean’s Certification Request form.

Students must complete the form and provide a signed release stating that the Office of Student Conduct has your express permission to release your conduct history to another party.
