Our online patient portal offers a convenient way to submit the health forms required by St. John's University in accordance with New York State Public Health Laws 2165 and 2167 to be an enrolled student.
Are You a New Student?
If you have deposited as a St. John's student or if you are registered for classes at St. John's, please submit your required health forms via Medicat, the online patient portal.
Health Form Submission
Once you sign on to Medicat, you will be able to upload your health forms.
To access your online portal account:
- Sign in using your St. John's network credentials at signon.stjohns.edu.
- For more information, please visit stjohns.edu/IT.
- Please do not create a new account in Medicat.
- Click on the icon for Medicat.
- Once in Medicat, you will be able to access and upload your health forms.
To download your forms:
- Download and print the required health form from the “Home” tab
- Have necessary documents completed, signed, and stamped by your health care provider
To upload your complete forms:
- Click on the “Upload” tab
- Review the Instructions provided
- To the bottom of the page, select the type of document you will be uploading, e.g., Immunization History
- Click “Select File” to browse your computer for the document.

Please note:
- Uploaded documents will not be considered compliant until reviewed by Student Health Services.
- As with any other personal identifying information on the Internet, always log out of the Online Patient Portal and close your internet browser when you are done using the portal to ensure your medical information remains confidential.
