Distinguished Practitioner Award Nomination

The College of Pharmacy and Health Sciences 2019 Gala nominations deadline is June 8. Any nominations that are received after June 8, 2018 will be considered for future award programs.

Recipient of the award will have demonstrated service, dedication and leadership in the profession.

Serve as an exemplary role model within his/her chosen profession
Demonstrate effective leadership skills, communication skills and commitment to the advancement of profession.
Develop creative teaching within a practice setting (if applicable).
Demonstrate program growth and sustainability over time.
Encourage interdisciplinary relationships with other healthcare professionals.
Demonstrate and encourage life-long learning
Provide mentoring and professional guidance
Actively participate in professional organizations
A minimum of 10 years of service
