Completing this form is one way that members of the St. John’s community can share or report experiences or concerns regarding incidents of bias, discrimination and/or harassment that may occur within the university community. You may also contact Human Resources directly by calling 718-990-2660.

Please take note that these instances do not need to specifically relate to one of SJU’s existing policies, and that using this form does not replace or reduce the reporting options in any of the University’s existing policies. St. John’s University expects all members of the University Community to abide by the University’s policies. Any potential violations of the University’s policies will be investigated.

Once you have completed and submitted this form, a designee of the University may contact you for clarifying information as appropriate, and to determine the appropriate action for response and resolution.

Bias-related incidents include those actions that are motivated by bias, but do not meet the necessary elements required to prove a crime.

Bias incidents are defined as: A verbal or physical action committed against or directed toward a person or to a person’s property because of the person’s actual or perceived race, color, national or ethnic origin, sex, sexual orientation, gender, gender identity and gender expression, disability, religion, age, alienage or citizenship status, veteran status, or any other legally protected basis. Bias incidents may include, but are not limited to, name calling, use of epithets, slurs or degrading language directed toward the targeted person or group, vandalism, symbols and other offenses against property involving graffiti or degrading images, threats or harassment.

Hate crimes are also motivated by bias with an accompanying crime, such as: threats of violence, property damage, personal injury and other illegal conduct.
