PLUS Loan and who would like to make a change to the dollar amount
of the loan or who would like to cancel all or part of their

Form
For graduate or law students who are enrolled for at least six
credits over the summer and who wish to apply for a Federal Direct
Graduate PLUS loan.

For students who would like to make a change to the dollar amount
of their Federal Direct Student Loans or who would like to cancel
all or part of their Federal Direct Student loans.
Non-Custodial PLUS Addendum
Non-custodial parents who wish to apply for a Federal Direct PLUS
loan must visit www.StudentLoans.gov to
complete the Federal Direct PLUS application and credit review
process and must also complete a non-custodial addendum.
