Below please find a listing of available online loan forms that will be of assistance to you in your direct loan process.

2013-2014

  • Federal Direct PLUS Loan Change Form
    For parent borrowers who have been approved for a Federal Direct PLUS Loan and who would like to make a change to the dollar amount of the loan or who would like to cancel all or part of their Federal Direct PLUS loan. 
     
  • Federal Direct PLUS/Graduate PLUS Loan Summer Request Form
    For graduate or law students who are enrolled for at least six credits over the summer and who wish to apply for a Federal Direct Graduate PLUS loan.
     
  • Student Loan Change Form
    For students who would like to make a change to the dollar amount of their Federal Direct Student Loans or who would like to cancel all or part of their Federal Direct Student loans.

Non-Custodial PLUS Addendum
Non-custodial parents who wish to apply for a Federal Direct PLUS loan must visit www.StudentLoans.gov to complete the Federal Direct PLUS application and credit review process and must also complete a non-custodial addendum.
