Re:New Personnel Change Form

In our continuing effort to improve internal processes and based on feedback from a number of managers, we have been reviewing many of our Human Resources forms and procedures. Toward this end, we have decided to revise the current Personnel Change Form (PCF).

As many of you know, the PCF is used to communicate a variety of changes to an employee’s HR record. The employee’s supervisor or department head completes the form. The completed form is then submitted by the department manager to HR for processing. The new PCF is designed to improve and simplify that communication process. In addition, the new PCF will replace the notice of employment (which has already been replaced by an offer letter for administrators and staff) for new student, Graduate Assistant, and temporary hires. We believe this change coupled with the recently announced new hire processing enhancements will reduce processing time and improve workflow.

We are sure you will find this new form easy to use.  However, if you have any questions on how to complete the form, please do not hesitate to contact us for assistance.

Please read the following instructions and procedures for the new PCF. Be sure to retain these instructions for your reference.
