Outpatient mental healthcare is available when medically necessary
and authorization is required through our Behavioral Health
Department. The benefit is covered for 30 visits per calendar
year. If you are being treated for a biologically based
condition you can exceed the 30 visits maximum, but visits for a
biologically based condition do apply to the visit maximum.
Coverage in network is a $10.00 copay per visit. Coverage out
of network is subject to deductible and coinsurance and usual and
customary allowances.
