Flexible Spending Forms

Flexible Spending Claim Form
This form should be completed to receive reimbursement for HealthCare and Dependent Care Flexible Spending expenses. This form may be faxed or mailed to P&A Group at the number and address listed at the bottom of the form.
Direct Deposit Form (Flexible Spending)
This form should be completed if you would like your Flexible Spending reimbursement to be directly deposited into a bank account. This form should be submitted directly to P&A Group at the address listed on the bottom of the form.
Request for Additional Debit Card
This form should be completed if you would like an additional debit card for other family members, or if you have lost your Flex debit card. This form should be submitted directly to P&A Group at the address listed on the bottom of the form.
