Required Forms

There are 6 documents that OGS needs in order to allow you to study abroad, as listed below. That may seem like a lot, but in fact, they take just minutes to complete.  The only exception is the flight information form, which requires that you already have purchased your flight. We love to receive forms early, so get started today!

1. Confirmation Form

Due Date: ASAP–programs fill quickly!
Use this form to confirm, deny or withdraw your admission from study abroad.  Make sure you are familiar with our Refund Policy before you confirm your participation.

2. Student Agreement (Assumption of Risk & Release)

Due Date: December 11, 2009
A standard release, which includes an authorization for medical treatment if necessary.

3. Health Disclosure Form

Due Date: December 11, 2009
Please use this form to share with us any relevant medical history, medical conditions, or special circumstances that require accommodation.

4. Roommate Questionnaire Form

Due Date: December 11, 2009
This form will help us place you with a suitable roommate. If there is someone that you already know you would like to live please indicate that on the form as well.

5. Flight Information Form

Due Date: December 11, 2009
Provide us with your flight information so that we know when to expect your arrival.

6. Photocopy of Passport

Due Date: December 11, 2009
Submit a photocopy of the picture page of your passport. We use this to help you in the unlikely event that your passport is lost or stolen while abroad.
