Survey Request Form
In order to assure that our survey activities are effectively
coordinated, that results are reliable and that efforts are not
redundant, the Office of Institutional Research will review all
proposed surveys. If you are planning to administer a survey,
please complete this Survey
Request Form and submit a copy of the survey at least two
weeks prior to administering the survey.
